Anyone interested in their children participating in our Telehealth program please complete the Telehealth consent form attached below and return to the school. Our Telehealth program is associated with the Pediatric Group (Dr. Reddy & Dr. Gerwe). This program allows the child to remain at school, the parent/guardian to remain at work or home and the child is able to have a doctors visit at school in the nurses office. We are able to test for covid, flu and strep as needed and doctors notes will automatically be provided to the school. If a prescription is needed, that prescription will be called into the pharmacy of your choice and will be ready for you to pick up before the end of the school day. The Pediatric Group DOES NOT have to be your child's primary care provider in order to use this service. This is a great opportunity and we are happy to be able to provide this to our students. School physicals & immunization records need to be submitted to the school no later than Oct. 15th. Vision exams need to be submitted to the school no later than October 15th.
Dental exams need to be submitted to the school no later than March 3rd. If you are planning to use Dental Safari this year please let me know. Dental Safari will be coming to the school on Feb 26, 2025. I will send home Dental Safari paperwork closer to time.

**ATTENTION SENIORS CLASS OF 2025**
SIU Carbondale: Starting Sept 3rd-Oct 31st, there will be no fee to apply to SIUC. Apply early. 3.8 unweighted GPA or higher will qualify students automatically for the University Excellence scholarship of $5,500. Watch your email for an invitation at that time to apply for the Chancellor scholarship, which is a full-ride. Automatic scholarships start at a 2.75 gpa that qualifies you for $1,500 per year renewable for 4 years and go up from there.
Students that have parents that are alumni of SIUC will receive 20% off tuition
Employees at SIUC for over 7 years can qualify their student for an additional 50% off
Families making below $103,040 will qualify automatically for the Saluki Commitment, which waives all tuition and fees.
